Tirzepatide is a novel therapeutic agent classified as a Dual Glucose-Dependent Insulinotropic Polypeptide (GIP) and Glucagon-Like Peptide-1 (GLP-1) Receptor Agonist. It is often referred to as a "twincretin" because it mimics the action of two natural gut hormones (incretins).
The dual action leads to enhanced metabolic control through several pathways:
- Glucose-Dependent Insulin Secretion: Tirzepatide stimulates the pancreatic beta cells to release insulin only when blood sugar levels are elevated. This effect reduces the risk of severe hypoglycemia when used alone.
- Glucagon Suppression: It reduces the secretion of glucagon (a hormone that raises blood sugar) from the alpha cells of the pancreas, which helps decrease hepatic glucose production.
- Gastric Emptying: It slows down the rate at which food leaves the stomach (gastric emptying). This contributes to a feeling of early and prolonged fullness (satiety).
- Appetite Regulation: By acting on receptors in the brain, it decreases appetite and reduces food intake, leading to significant body weight loss.
Tirzepatide is administered as a subcutaneous injection once weekly.

- Frequency: Once a week, on the same day each week, at any time of the day, with or without meals.
- Injection Sites: Abdomen, thigh, or upper arm. Rotate the injection site each week.
- Missed Dose: If a dose is missed, administer it as soon as possible within 4 days (96 hours) of the missed dose. If more than 4 days have passed, skip the missed dose and resume the regular weekly schedule.
Proper storage is critical to maintain the drug's efficacy.
